Electronic Authorization & Consent Agreement for Background Checks
I, the applicant, authorize Windy Lease & Rental and its designated agents and representatives to conduct a review of my background for rental eligibility, which may include:
- Criminal history checks
- Employment history verification
- Educational background verification
- Credit history checks
- Professional license verification
- Reference checks
I knowingly authorize companies, credit bureaus, public agencies, and other lawful sources to release information requested by Windy Lease & Rental or its authorized representatives for evaluating this application. I release the parties providing and receiving this information from liability to the extent permitted by law.
I consent to the procurement of consumer reports and investigative consumer reports during the application process and, where permitted, during a rental or business engagement. I understand that a personal or business credit report may be obtained with this consent.
Information obtained will be handled confidentially and shared only with people who have a legitimate business need, subject to applicable law. This authorization remains valid during the application process and, if approved, during the applicable rental or business engagement.
Final requirements will be confirmed in the rental agreement. Typical requirements may include commercial general liability coverage of at least $1,000,000 per occurrence and $2,000,000 aggregate, applicable auto liability coverage, and naming the lessor as an additional insured and loss payee where required.
